Melasma—A Discoloration on the Face and Forehead

Melasma is the discoloration commonly found on the upper cheek, nose, lips, upper lip, and forehead in women and men. It is also known as "Chloasma faciei" or the mask of pregnancy when present in pregnant women. It is a tan or dark skin discoloration. Although it can affect anyone, melasma is par ticularly common in women, especially pregnant women and those who are taking oral or patch contraceptives or hormone replacement therapy (HRT) medications. It is also prevalent in men and women of Native American descent (on the forearms) and in men and women of German/Russian and Jewish descent (on the face).

The symptoms of melasma are dark, irregular patches commonly found on the upper cheek, nose, lips, upper lip, and forehead. These patches often develop gradually over time. Melasma does not cause any other symptoms beyond the cosmetic discoloration.

Melasma is thought to be caused by the stimulation of melanocytes or pigment-producing cells by the female sex hormones estrogen and progesterone to produce more melanin pigments when the skin is exposed to sun. Women with a light brown skin type who are living in regions with intense sun exposure are par ticularly susceptible to developing this condition. The discoloration usually disappears spontaneously over a period of several months after giving bir th or stopping the oral contraceptives or hormone replacement therapy.

Genetic predisposition is also a major factor in determining whether someone will develop melasma.

The incidence of melasma also increases in patients with thyroid disease. It is thought that the overproduction of melanocyte-stimulating hormone (MSH) brought on by stress can cause outbreaks of this condition. Other rare causes of melasma include allergic reaction to medications and cosmetics.

Melasma Suprarenale (Latin—above the adrenals) is a symptom of Addison's disease, particularly when caused by pressure or minor injur y to the skin, as discovered by Dr. FJJ Schmidt of Rotterdam in 1859.

Melasma is usually diagnosed visually or with assistance of a Wood's lamp (340 - 400 nm wavelength). Under Wood's lamp, excess melanin in the epidermis can be distinguished from that of the dermis.

There are many treatments to hasten the fading of the discolored patches such as topical depigmention agents; an acid that increases skin cell turnover; one that decreases the activity of melocytes; facial chemical peels, laser treatments and intense pulsed light.

At Miramar Dermatology, the physicians per form the Melanage Peel. "The Melanage system has revolutionized the way we treat our patients with melasma. Our success rate is much better now that we have this modality" says Dr. Gottlieb.

-By Anita Finley

Listen to Dr. Gottlieb and Anita Finley on Saturday, September 11th from 7:00-7:30 AM on WSBR 740AM and on the Internet at www.wsbrradio.com

Is Your Acne Care Routine getting in the way of your busy schedule?

According to a new study published in the July 2010 issue of Journal of Drugs in Dermatology, an Acne care regimen does not have to eat away at your precious time.

Researchers have found that a treatment regimen including Retin-A Micro® Pump can deliver effective results while cutting everyday treatment time without sacrificing results. This once-daily treatment taken in the morning along with benzoyl peroxide has been shown very effective.

These newly published study results are significant for both acne patients and dermatologists as they reinforce the ability to develop skin care regimens that cater to individual patients' lifestyles. This will encourage patients, particularly teens, to follow their prescribed treatment regimen.

Retin-A Micro® Pump is the only acne medication to combine tretinoin, the number one prescribed retinoid, with patented microsphere technology. The microspheres gradually release tretinoin over time to reduce irritation and help absorb excess facial oil to reduce facial shine. It is now available in a unique, sleek pump that delivers a consistent amount of medication, making it easy to use and less likely to over use.

It is important to remember that severe Acne is a medical condition and treatment must be administered by a medical professional.

For more information about the Retin-A Micro ® Pump, or other treatments for acne and skin conditions, contact the professionals at Miramar Dermatology Associates. Our Doctors will be happy to answer any questions you may have and determine which treat is right for you or you’re teen.

South Floridian Cold Weather, Dry Skin, Chapped Lips?

It’s not very often that we in South Florida experience winter weather. In recent weeks, the air outside has been cold and lacking the normal humidity associated with Florida. This year many of us actually turned on the heat for the first time in a long while, which makes the air dry and can create a perfect environment for chapped skin and cracked lips.

Since many of us have never lived with cold weather conditions, or maybe have forgotten how to deal with these conditions, the doctors at Miramar Dermatology thought it would be a good idea to give you some advice to keep your skin healthy and viverant.

To prevent scaling, flaking and itching, pay special attention to your skin during the cold months by applying a cream-based moisturizer every day. A good time to use moisturizer is just after showering to help trap moisture on the skin's surface. This will aid in keeping your skin moist throughout the day.

We all know that washing hands often is imperative in the prevention of the spread of germs; however, all of that soap and water can make your hands chapped. You should consider using a mild soap and moisturizing afterwards to relieve the dryness.

When it’s cold nothing feels better than a hot shower or bath, however water that is too hot can actually do more damage to the skin than good. Short showers or baths with lukewarm water and a non-irritating, non-detergent-based cleanser are easier on the skin. Immediately afterward, apply a mineral oil or petroleum jelly-type moisturizer and gently pat skin dry.

A humidifier can help to deal with dry air from heaters; this will add some moisture back into the air. Don’t forget to clean your humidifier often to prevent mold and fungus.

The doctors at Miramar Dermatology want you to remember that scaling, itching or cracked skin that is not relieved by moisturizers should be checked by a dermatologist, who may suggest a prescription medication.
